Online Order Form

All cards are a heavy 16pt. matte card stock and all are double sided.

Pricing Options: 1000 cards: $50 500 cards: $40.
You will choose your preferred quantity during checkout.

This order form requires payment made through PayPal. If you do not have a PayPal account, it's free and easy to sign up. Click here to visit PayPal and set up an account. If you would rather send a money order by mail, please click here.

Please fill out the following form. When inputting your information. BE SURE TO DOUBLE CHECK your information, as it will print on your card exactly as entered in the form.

After submitting your form, you will be redirected to PayPal to complete your order. Your order is not complete until you finalize your payment.

Requirements:
You may not have clubs on your business card. You must use your school email address, not personal addresses.
You can not include social networking sites such as Twitter, Facebook, etc.

This first part of the form is for our records. It will allow us to contact you if needed. Use the email address and phone number that you will best be reached at. For example, your gmail email as opposed to your school email address.

The below information is what will appear on your business card. Make sure all of the information is correct before submitting.

NOTE: If you decide to not include a field on your business card, (i.e you home address or phone number), you must write the word "none"
in each field you wish not to include on your business card.
